Overview

The Executive Medical Assessment provides busy executives and clients with an efficient way to receive a confidential, comprehensive head-to-toe assessment and personalized care.  It is a one-day experience, with approximately 5.5 hours spent at our location and is performed annually.

Our EMC dedicated staff will schedule a series of examinations and diagnostics through EMC’s onsite independent medical affiliates, review the reports and then deliver a summary of tangible, actionable results, all in a single day.

  • Based on the results, an action plan will be discussed, with a report to follow with detailed results within 7-10 days of the appointment date.

What to Expect

Our dedicated team including our Executive Patient Coordinator and Nurse Practitioner will be your key contacts and will provide you with exceptional one-on-one service.

Prior to the Scheduled Appointment:

  • You will be asked to complete an enrollment form with your contact details. This form will also provide the terms and conditions of the Executive Medical Assessment program
  • We will then send a welcome email together with a medical questionnaire to the enrolled executive/client which allows our medical team to review all past medical history prior to the appointment date
  • Upon registration in the program, clients will be provided with the contact details for their assigned Executive Patient Coordinator
  • Your employer / payer will be invoiced the full cost plus HST of the Executive Medical Assessment
  • Our Executive Patient Coordinator will then arrange the date and time of your Executive Medical Assessment and forward an introductory package which will include all patient preparation details
  • EMC will arrange with the lab to have blood work drawn at the executive’s preferred location (including home or office, subject to certain restrictions), approximately one week prior to the appointment date. This ensures a full review takes place at the end of the day, rather than a partial review with lab results to be discussed after the fact

Appointment Day

You will be greeted by our Executive Patient Coordinator in our main lobby and enjoy refreshments in our waiting area. You will then meet with our medical practitioner (either nurse practitioner or physician) to review your medical questionnaire, additional medical history, medications and be provided a head-to-toe assessment.

Our complete agenda will comprise medical services/diagnostic tests, a dedicated lounge with WIFI, a mid morning snack, lunch and a waiting area between appointments. Our Executive Patient Coordinator as well as a member of our medical team will be available throughout the testing.

If follow-up specialist medical appointments or diagnostic testing/procedures are recommended by the Executive Medical Assessment report, they will be referred to the client’s family physician for proper follow-up. EMC is not responsible for the client’s ongoing medical care.

Limitation of Services

EMC provides no healthcare services, but rather is a health assessment service for its clients.  Although EMC may contract/arrange appointments with a variety of healthcare entities, including physicians, nurse practitioners, and diagnostic clinics, these providers provide healthcare as independent contractors, with their own insurance company coverage (e.g. for physicians CMPA).

The EMCs role is strictly the arrangement of a one-time health assessment service.

Terms and Conditions

  1. The services provided by Executive Medical Concierge Canada (2021) Ltd. (“EMC”) are scheduled executive assessment(s) coordinated through EMCs Executive Patient Coordinator or Nurse Practitioner.
  2. EMC provides annual Executive Assessment programs as well as Executive Medical Navigation Services. EMC is not an emergency medical service. In the event that you are experiencing a medical emergency please call 911 or proceed directly to the emergency department of your nearest hospital.
  3. EMCs Executive Assessment Program is a one day pre-scheduled visit, which is recommended annually. The fee includes one Executive Assessment visit only.
  4. EMC Executive Assessment appointments are reserved for each of EMCs clients with specialized medical professionals at specific times. If a client needs to change an appointment or cancel, all changes made within less than 10 days prior to your appointment will be subject to a service fee automatically charged to your company. Changes within 4-10 business days before your appointment are subject to a rescheduling fee of $50 or cancellation fee of $250. Changes within 1-3 business days before your appointment are subject to a rescheduling fee or cancellation fee of $295. If lab work has been performed prior to any cancellation a fee of $300 will be charged in addition to the cancellation fee to cover the cost of the lab work.  Same day changes and no-shows will be charged the full amount.
  5. EMC shall be responsible for the coordination of your annual Executive Assessment only. EMC provides no health care services. Services will be arranged as needed by EMC through independent health care provider/s, with follow-up to be provided by the client’s family physician. For greater certainty, but without limiting the foregoing, the following are not covered by your Executive Medical Assessment and are the client’s sole responsibility following the Executive Assessment day: the cost (within or outside of Canada) of medical/health care of any kind (unless covered by your Provincial Health Plan), paramedical care, home care, diagnostic testing, procedures, prescription costs, medico-legal letters/services, insurance assessments/letters, and travel costs.
  6. If follow-up specialist medical appointments or diagnostic testing/procedures are recommended by the Executive Medical Assessment report, they will be referred to the client’s family physician for proper follow-up. EMC is not responsible for the client’s ongoing medical care.
  7. EMC cannot arrange for the signing of referrals recommended by other physicians, homeopaths, naturopaths or other alternative doctors. Such signature is up to the discretion and independent judgment of the appropriate professional in accordance with accepted medical principles.
  8. Services provided by EMC are limited to the client named on the enrollment form only.
  9. Clients enrolling in the EMC Executive Assessment program should stay with their family physician. EMC services are complementary to the healthcare system.
  10. EMCs Executive Medical Navigation program is not included as part of the Executive Assessment. Clients interested in EMCs Executive Medical Navigation should contact our Executive Patient Coordinator.

Cancellation Policy

EMC Executive Medical Assessment appointments are reserved for each of EMCs clients with specialized medical professionals at specific times. If a client needs to change an appointment or cancel, all changes made within less than 10 days prior to your appointment will be subject to a cancellation/service fee automatically charged to your company/ the individual (depending on enrollment details). In particular:

  • Changes or cancellations with more than 10 days prior notice are subject to no cancellation/service fee
  • Changes within 5-10 business days before your appointment date are subject to a rescheduling fee of $100 or cancellation fee of $250. If lab work has been performed prior to any cancellation, a fee of $300 will be charged in addition to the cancellation fee to cover the cost of the lab work.
  • Changes within 2-4 business days before your appointment are subject to a rescheduling fee or cancellation fee of $295. If lab work has been performed prior to any cancellation a fee of $300 will be charged in addition to the cancellation fee to cover the cost of the lab work.
  • Changes with less than 24 business hours’ notice and no-shows will be charged the full amount.
