Executive Medical Concierge (EMC) initially opened in 2013 in response to concerns from patients regarding the difficulties they were facing navigating the healthcare system, both locally and abroad. The first program offered by EMC was the Executive Medical Navigation program, which provides one point of contact (our Executive Patient Coordinator or Nurse Practitioner) who coordinates, books and consolidates all necessary healthcare appointments (medical specialist consultations, laboratory testing, diagnostic imaging, and paramedical services etc.) with the members’ convenience in mind. Registered clients have the advantage of personal case management with direction to the best treatment options together with after-hours telephone access to a health professional. All medical arrangements are overseen by a supervising physician.

Based on positive feedback from clients who utilize our Executive Medical Navigation program, EMC has recently introduced its Executive Health Assessment program, which complements the navigation services offered by EMC. The Executive Health Assessment program utilizes the services available at the adjacent Polyclinic medical specialty and diagnostic infrastructure to provide a North Toronto option for the executive physical that has been traditionally provided downtown. Instead of taking a full day downtown, we offer a 5.5 hour (approximately) appointment under one roof at a convenient location, with access to quality assessments by top service providers in the health care field.

Our Facilities

Both Executive programs are housed adjacent to the Polyclinic Family and Specialty Medicine Facility, which is a comprehensive state-of-the-art 30,000 square foot medical facility located within the Champagne Centre Health Hub located at 2 Champagne Drive in North York (Dufferin St. and Finch Ave. W.) with free parking and convenient TTC accessibility).

The Champagne Centre Health Hub, in tandem with Polyclinic, currently house the services of over 65 physicians (17 family physicians and over 50 specialists in a wide array of areas) practicing on a full-time or part-time basis, many with affiliations at major Toronto and surrounding hospitals. Also offered at the site are comprehensive diagnostic facilities including but not limited to laboratory, x-ray, ultrasound, bone-density, mammography, full pulmonary function testing, methacholine challenge testing, endoscopy, sleep studies, stress testing, echocardiography, stress- echocardiography, holter monitoring, ambulatory blood pressure monitoring, diet and exercise counseling, and audiology testing. We also have an onsite pharmacy, rehabilitation centre, dentists, pain clinic, surgical eye centre and optometrist, all under one roof.

Our Team

For the Executive Medical Navigation program, our Executive Health Team includes our Executive Patient Coordinator, Nurse Practitioner, and Supervising Physician, who together will ensure seamless coordination (not provision) of healthcare services, such as specialist appointments, diagnostic testing, prescription renewals, home care services, patient education services, out of country health coordination, paramedical services, admission and discharge planning, all while serving as a liaison between the client and the healthcare provider/facility and family.

Our team will do our best to ensure that an appropriate level of health care is extended to our clients through our vast network of independent services providers.

For the Executive Medical Assessment program our Executive Health Team will guide the client through the approximately 5.5 hours of booked assessments/tests with independent healthcare practitioners. At the end of the day, under the guidance of the supervising physician, the client will be provided a summary health report, together with detailed results, which can thereafter be provided to the client and their family physician.
